Position: On Call Weekend RN - Full Time | Agape Hospice Pinal County

Job Summary

The registered nurse plans, organizes and directs hospice care with emphasis on community health education/experience. The professional nurse builds from community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.

Duties & Responsibilities
  • Patient Care
    • Completes an initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
    • Provides a complete physical assessment and history of current and previous illnesses.
    • Uses the nursing process: evaluates status, writes and initiates a plan of care, re‑evaluates regularly, and revises the plan as necessary.
    • Develops a care plan with nursing diagnosis, goals and palliative actions, involving the patient and family.
    • Administers medications and treatments as prescribed by the physician.
    • Counsels patients and families to meet nursing and related needs, providing health care instructions per assessment.
    • Assists patients with activities of daily living and facilitates self‑sufficiency and optional comfort care.
    • Acts as case manager, coordinating patient care for an assigned caseload.
  • Communication
    • Maintains accurate clinical notes regarding patient condition and care.
    • Records pain/symptom management changes and outcomes.
    • Communicates with the physician on patient needs, reports changes, and obtains orders.
    • Communicates with community health persons to coordinate the care plan.
    • Teaches patients and families self‑care techniques and medication, diet, and other instructions as ordered.
    • Works in concert with the interdisciplinary group and maintains a safe environment for patients.
    • Provides continuity of care with family/caregiver and hospice interdisciplinary group members to meet emotional needs.
    • Attends interdisciplinary group meetings.
  • Additional Duties
    • Participates in on‑call duties as defined by the policy.
    • Ensures arrangements for equipment and necessary items and services are available.
    • Supervises ancillary personnel and delegates responsibilities when required.
    • Maintains and upgrades professional knowledge through continuing education and in‑service classes.
    • Participates in quality assessment, performance improvement teams and activities.
Job Requirements (Education, Experience, Knowledge, Skills & Abilities)
  • Graduate of a National League for Nursing accredited school of nursing.
  • Minimum of two years of experience, at least one year in public health, home care, or hospice nursing preferred.
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Responsible for supervising hospice aides.
  • Demonstrates excellent observation, problem‑solving, verbal and written communication, and nursing skills per competency checklist.
  • Shows ability to organize and prioritize workload independently.
  • Prolonged walking, standing, lifting, positioning, and transferring patients; lifting supplies and equipment.
  • Visual acuity and hearing sufficient for required nursing skills.
  • Maintains a current CPR certification.
  • Possesses valid driver’s license and automobile insurance; essential for agency‑related travel.
  • Self‑directed; works with minimal supervision.
