Field Nurse Practitioner​/Physician Assistant

Field Nurse Practitioner / Physician Assistant – up to $150,000/year + bonus!

Health Care Support is seeking a Field Nurse Practitioner or Physician Assistant to join a rapidly growing healthcare provider serving patients in Casa Grande, AZ. This is a unique opportunity to deliver high‑quality, patient‑centered care directly in the home—focusing on Chronic Kidney Disease and primary care needs. Monday – Friday, 8:00 AM – 5:00 PM

$140,000 – $150,000 annually + 10% annual bonus

• Mileage reimbursement provided

• Conduct comprehensive in‑home patient visits, including history and physical exams
• Develop and implement individualized treatment plans
• Order and interpret diagnostic tests and lab work
• Prescribe medications as appropriate
• Provide ongoing patient education and chronic disease management
• Complete accurate and timely documentation of services
• Collaborate with interdisciplinary care teams and participate in routine meetings
• Utilize telehealth to support patient care when appropriate

• Active Nurse Practitioner (NP) or Physician Assistant license in Arizona
• 2+ years of experience as a practicing NP or PA
