At the Community Health Center of Central Wyoming (CHCCW), we believe that behavioral health is an essential part of whole-person care. We're seeking a compassionate, collaborative, and dedicated Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join our integrated healthcare team and help improve the lives of individuals and families throughout our communities.
At CHCCW, you'll work alongside medical providers, behavioral health professionals, and care teams to deliver comprehensive, patient-centered care. You'll have the opportunity to build meaningful relationships with patients while providing counseling, case management, crisis intervention, and treatment planning in a supportive and mission-driven environment.

What You'll DoAs a member of our multidisciplinary healthcare team, you will:
- Provide comprehensive behavioral health assessments, diagnostic interviews, and biopsychosocial evaluations.
- Develop individualized treatment plans and provide evidence-based counseling services.
- Diagnose behavioral health conditions using DSM-5 criteria.
- Provide individual and, when appropriate, group counseling services.
- Assist patients experiencing anxiety, depression, ADHD, substance use disorders, relationship concerns, trauma, and other behavioral health needs.
- Coordinate care and case management to ensure continuity of services.
- Collaborate closely with primary care providers and other healthcare professionals to integrate behavioral and physical healthcare.
- Provide age-appropriate behavioral health services for pediatric, adolescent, adult, and geriatric patients.
- Maintain accurate, timely, and confidential clinical documentation.
- Support compliance with CHCCW clinical policies, procedures, and quality standards.
- Participate in departmental meetings, case consultations, and quality improvement initiatives.

- Master's Degree in Social Work or Master’s Degree in Counseling from an accredited institution.
- Current Wyoming Licensed Clinical Social Worker (LCSW)/Licensed Professional Counselor (LPC) license or license-eligible in Wyoming.
- Experience working in a medical or integrated healthcare setting preferred.
- Experience conducting biopsychosocial assessments and developing treatment plans.
- Experience providing counseling across diverse populations and age groups.

This position requires prolonged periods of sitting, computer work, and documentation, with occasional standing, walking, bending, lifting, and travel between clinic locations. Work is performed primarily in a clinical setting and may involve exposure to blood borne pathogens and infectious diseases. Appropriate personal protective equipment is provided and required.
