Qualifications

Degree from an accredited Occupational Therapy Program.

Home Health experience is a plus.

Current and unrestricted OT licensure.

Current CPR certification.

Good organizational and communication skills.

Valid driver’s license, auto insurance, and reliable transportation.

Job Overview

We are seeking a compassionate and skilled Occupational Therapist to join our home health team. In this role, you will provide one-on-one rehabilitative care to patients in the comfort of their homes, helping them regain independence, improve functional abilities, and safely navigate daily living tasks. The ideal candidate brings strong clinical judgment, excellent communication, and a commitment to delivering high-quality, patient-centered care.

Responsibilities include conducting evaluations, developing individualized care plans, implementing therapeutic interventions, and collaborating closely with the interdisciplinary team to support optimal patient outcomes. This position offers autonomy, meaningful patient relationships, and the opportunity to make a direct impact on each patient’s quality of life.

As a Home Health Occupational Therapist, you will:

Assess and Develop Programs

Evaluate patients' daily living and work-related skills, developing therapeutic retraining programs with measurable objectives. Administer and interpret diagnostic tests to assist the physician in evaluating patients' functional levels.

Collaborate on Treatment Plans

Work with physicians and healthcare team members to develop and revise Plans of Care.

Provide Therapeutic Treatment

Administer therapeutic treatment and instruction to patients according to physician orders to improve or restore strength, coordination, range-of-motion, and function, or teach compensation measures.

Enhance Retraining Programs

Review and expand retraining programs to align with patients' behavioral goals and the overall rehabilitation program.

Consult and Coordinate

Collaborate with vocational team members to develop and implement vocationally oriented plans tailored to patients' needs and capabilities.

Report and Document

Report patients' status and progress to physicians, rehabilitation staff, Clinical Managers, and family members. Accurately and thoroughly document patient care observations, interventions, and evaluations on the day services are rendered. Ensure interim (verbal) orders are accurately documented and implemented. Submit evaluations, treatment plans, and discharge summaries to supervisors and care management staff.

Participate in Care Coordination

Engage in care coordination and discharge planning activities, acting as a resource for other healthcare personnel.

Design Community Activities

Create community reintegration activities to aid in physical reconditioning and psychological adjustment, coordinating plans with the interdisciplinary team.

Recommend Adaptive Equipment

Suggest and design special adaptive equipment to improve patients' home or work environments or enhance participation in rehabilitation programs and the community.

Train Staff and Family

Design and train staff and family members to implement retraining programs, including dressing, feeding, grooming, and hygiene skills. Participate in team and family educational meetings.

Supervise and Evaluate

Oversee and evaluate the performance of Occupational Therapy Assistants and Home Health Aides in implementing occupational therapy services.
