Position OverviewAs a Physical Therapist (PT), you will evaluate, plan, and implement individualized treatment programs using evidence-based practices to help residents improve mobility, strength, balance, and overall functional independence. You’ll also play a key role in patient and family education, interdisciplinary collaboration, and documentation of care.
Key Responsibilities- Evaluate residents to assess mobility limitations and therapy needs
- Develop and implement personalized treatment plans focused on strength, balance, gait, and functional mobility
- Monitor patient progress and adjust treatment plans as needed in collaboration with the care team
- Recommend assistive devices and interventions to enhance safety and independence
- Educate patients, families, and caregivers on mobility strategies and fall prevention
- Maintain accurate and timely documentation in compliance with state and federal guidelines
- Participate in interdisciplinary meetings and care planning
- Ensure compliance with all regulatory and facility policies
- Active Wyoming Physical Therapist license
- In good standing with all applicable licensing boards
- Experience in Skilled Nursing or long-term care preferred
- Familiarity with Net Health documentation system is a plus
- Strong communication and interpersonal skills
