This leadership role is responsible for directing the clinical operations of our hospice program while cross‑training to support home health services. Care is provided in the patient’s home, primarily serving an aging population. This position oversees a skilled and compassionate hospice team, ensuring high‑quality care in areas such as wound management, comfort-focused interventions, and educating patients and families on disease progression.
Support team with occasional on call help.
In addition, the role includes cross‑training to support our home health team, providing clinical oversight in a variety of skilled services. Areas include IV/infusion therapy, cardiopulmonary assessment, diabetic education, post‑surgical care, chronic disease management, medication reconciliation and management, gastrostomy tube care, injections, blood draws, and in‑dwelling catheter care and maintenance.
Responsibilities- Foster a supportive, professional environment that empowers team members to excel in their specialties.
- Partner with staff to develop individualized, patient‑centered plans of care that meet the unique needs of both patients and families.
- Lead and promote quality improvement initiatives, safety practices, and infection control standards across service areas.
- Serve as a clinical resource and guide for team members, ensuring compliance with regulatory standards and best practices.
- Contribute to a culture of excellence, compassion, and continuous improvement.
- Current active RN license in good standing with the state of Wyoming
- CPR certification3 years of clinical leadership, community health preferred
- Demonstrated ability to manage policies and procedures
- Demonstrated ability to build a team, fostering a positive culture for mentoring, learning, and growing
