Job Summary

We are seeking a highly motivated and experienced RN to provide exceptional patient care to our patients in the safety and comfort of their homes. As an RN with Home&Care, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide direct patient care, including assessment, planning, implementation, and evaluation of care for our residents
  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team
  • Administer medications, treatments, and therapies as prescribed by physicians
  • Monitor and document patient progress, identifying and reporting changes in condition
  • Communicate effectively with patients, families, and healthcare team members
  • Participate in quality improvement initiatives to enhance patient care and satisfaction
  • Collaborate with other departments to ensure seamless transitions and optimal care
Requirements
  • Current RN license in the state of Wyoming
  • Minimum years of experience: 0 years (new graduates welcome to apply)
  • Ability to lift, push, and pull heavy objects (up to 50 pounds)
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment with multiple priorities and deadlines
