Position Summary
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. The position is accountable for the quality of nursing services delivered by self or others who are under their direction. It utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.

Functions
Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Review diagnostics and laboratory data and report abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and policy.Formulates a plan of care, including the discharge plan, utilizing assessment data, patient, family and health team input. Initiates a plan based on patient-specific needs, assessment data and medical plan of care. Develops realistic, measurable goals in collaboration with the patient/family. Considers physical, cultural, psychosocial, spiritual, age-specific and educational needs of the patient. Plans care in collaboration with multidisciplinary team members and revises plan as needed.Implements the plan of care through direct patient care, coordination, delegation and supervision of the health care team. Provides care based on physician orders and the nursing plan, in compliance with policies, procedures, standards of care and regulatory requirements. Delegates appropriately, supervises other nurses and personnel, and promotes continuity of care through accurate communication.Evaluates the patient’s and family’s response to care and teaching, revising the plan as needed. Assesses patient progress towards goals and outcomes with the team and evaluates the effectiveness of teaching.Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and policy‑compliant, reflecting objective and subjective data, interventions and patient response.Provides care based on best evidence and may participate in research and shared leadership. Engages in education, orientation and mentoring of staff, patients, students, families and visitors.Contributes to society through activities that improve patient outcomes—timely, effective, efficient, equitable and safe care. Participates in quality indicator improvement and professional organizations.May supervise staff and workflow of the department.Interacts with all levels of staff, physicians, patients, families and external contacts to coordinate care and provide spiritual support via clergy.Minimum Qualifications
Must possess a current, valid RN license in the state of practice, temporary/interim RN license or compact RN licensure for the current state. BLS certification is required. Experience in the clinical area desired; must maintain competencies appropriate to the area. For acute care settings, a minimum of one year of RN experience in an acute care hospital within the past 12 months is required.
Preferred Qualifications Bachelor’s degree preferred. Professional certification preferred. Additional relevant education and/or experience preferred.
