Case Manager
We are seeking a dedicated Case Manager to join our program and support individuals in need of non‑clinical case management services.
Responsibilities- Provides non‑clinical case management support for soldiers requiring medical readiness assistance, ensuring accurate case tracking and documentation.
- Provides trauma‑informed, person‑centered support to individuals in substance use and/or mental health recovery who have a history of homelessness, assisting them in accessing needed resources.
- Supports program components and facilitates services for homeless individuals and families, including shelter services and community‑based services.
- Coordinates services with families, providers, and interdisciplinary teams to ensure quality care for residents.
- Assists the growing Personal Injury team in helping clients obtain necessary resources and support.
- Experience in client‑focused case management and/or customer account management.
- Strong communication skills and attention to detail.
- Experience with Medicaid services or related Home and Community‑Based Services (optional).
