Wyoming Independent Living is seeking a Community Choice Waiver Case Manager to empower individuals with disabilities to regain and maintain independence. You will coordinate in-home services, complete assessments, and support clients to set Independent Living Goals.
Travel within the designated service area is required; you will partner with consumers, families, and community partners to connect resources and advocate for consumer rights and options.
