• Hospice heart: compassionate, kind and empathetic to patients and their families
• Nurse/Patient Relationship:
  Develops and maintains a therapeutic relationship through the health care continuum
• Provide direct patient care services to hospice patients
• Empower caregivers by educating them on care for their loved one
• Provide coordination of care, ensure continuous assessment of each patient's and family's needs
• Responsible for documentation and communication of care, medication, and observations made during care of the patient in order to track responsiveness and health of patient
• Responsible for providing appropriate patient/family education to help patient/family achieve a pain‑free and comfortable status

• The Registered Hospice Nurse will manage pain and symptoms so patients can make the most of every day, focusing on living well and with dignity
• The Registered Hospice Nurse will provide compassionate care to patients and their family members at end of life

Base rate for being on call

$50-100 visit rate

• Registered nurse with current licensure to practice professional nursing in the state
• Graduate of National League for Nursing accredited school of nursing
• Maintains a current CPR certification
• Current valid driver’s license
• At least one year of Hospice experience (preferred)
• Demonstrates excellent observation, problem solving, verbal and written communications, nursing skills per competency checklist
• Shows ability to organize and prioritize workload independently
