Hourly Salary Range: $42.00 - $46.00 per hour

Annual Pay: $60,000 - $73,000

• Provide direct patient care under the supervision of a registered nurse.
• Conduct assessments, monitor patient conditions, and communicate findings to the healthcare team.
• Assist with personal care activities if needed.
• Offer emotional support and companionship to patients and their families.
• Collaborate with other members of the interdisciplinary team to develop and implement individualized care plans.
• Maintain accurate and up-to-date documentation of patient care activities, including clinical/progress notes.

• Preferably one (1) year of experience as a Licensed Practical Nurse with Hospice experience.
• Has completed a practical (vocational) nursing program and is licensed in good standing in the state of Colorado and furnishes services under the supervision of a qualified Registered Nurse.
• Must be able to communicate in the language of the client and document client‑related activity as appropriate.
• Possesses adequate and effective written and verbal communication skills.
• Possesses adequate and effective interpersonal skills.

Requires standing, walking, bending, kneeling, and stooping all day, and frequently lifting or moving items weighing up to 50 pounds.

Daily travel required; mileage reimbursed.
