Overview
Empatia Palliative Care is looking for a PRN Nurse Practitioner to cover our Castle Rock palliative care patients.
The Nurse Practitioner Palliative Care facilitates palliative care for Empatia Palliative Care patients, ensuring a comprehensive, individualized treatment plan, while demonstrating excellent clinical skills and ethical, relevant judgment. Provides diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director (if required under State law).

- Full prescriptive Authority, Experience Writing - Schedule
- Looking for someone at least 1-2 days/week - Palliative Care/Hospice experience a plus
Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.
Hourly Range - $57-65/hour
About YouSpecialized Knowledge and
Skills:
Strong leadership and interpersonal communication skills.
Independent decision-making ability and self-motivation.
In-depth knowledge of palliative medicine principles, Medicare/Medicaid regulations, value-based care, and interdisciplinary processes.
Meets continuing education requirements set by Gentiva and licensing boards.
Proficiency with electronic medical record (EMR) software.
Willingness to travel 10-20% for training and education.
Education and Experience:
Master of Science in Nursing (MSN) or equivalent from an accredited institution.
Minimum two years of Nurse Practitioner clinical experience.
Specialty in Family or Adult Medicine preferred.
Experience with value-based care models and Part B billing preferred.
Ability to work autonomously in an advanced practice role.
Licenses, Certifications, and Agreements:
Current RN license or equivalent as required by state law.
- Active and Unrestricted Nurse Practitioner certification/license without board actions
Collaborative agreement with supervising physician from the Professional Services Corporation.
Part B Provider certification.
Federal DEA license; state DEA license if applicable.
ACHPN certification preferred.
Valid driver’s license and current automobile insurance.
Technical Requirements:
Fluent competence in computer and smartphone navigation.
Working knowledge of Microsoft Outlook, Word, Excel, and PowerPoint.
Reliable transportation required.
