Job DescriptionProvides nursing care and guidance to patients from initial contact through the continuum of care. Guides patients on treatment plans, next steps in appointments, and assists with scheduling follow-up appointments. Tracks outcomes of referrals and treatments, maintaining detailed records to measure program effectiveness. Develops and implements patient education programs and tools. Provides psychosocial support, including facilitation of support groups. Educates and facilitates patient introduction to clinical trial information, resource referrals, and counseling.
Advocates for patients and families in preventing and resolving clinical issues. Develops strong relationships with physicians, office staff, community resources, and healthcare providers to ensure effective communication in patient care. Serves as a liaison between clinical specialists, primary care physicians, and patients. Coordinates and participates in community education programs, health fairs, and local events. Provides formal and informal education to other healthcare team members to achieve desired outcomes.
Attends national, regional, and local conferences pertinent to the program and the RN Navigator role. Other duties as assigned.

- Proficiency in Microsoft Office (Outlook, Excel, Teams) and electronic medical record systems
- Bilingual abilities (English/Spanish) [Preferred]
- Bachelor's of Nursing [Required]

- 1+ years in acute care nursing [Required]
- 1+ years in an oncology setting [Preferred]
- Oncology nursing experience [Preferred]
- N/A
- Registered Nurse (RN) [Required]
- Basic Life Support – CPR Cert (BLS) [Required]
Physical Requirements -
Pay Range$35.04 - $65.17
