• Perform comprehensive patient evaluations to assess functional performance, self-care skills, and rehabilitation needs
• Develop and implement personalized treatment plans tailored to each patient’s goals and clinical needs
• Provide therapeutic activities, exercises, and adaptive strategies to maximize independence
• Monitor and document patient progress while ensuring compliance with state and federal guidelines
• Educate patients and families on therapy goals, adaptive equipment, and home exercise programs
• Collaborate with the interdisciplinary healthcare team to coordinate high-quality patient care

• Active and current Occupational Therapist (OT) license in the state of California (required)
• Graduate of an accredited Occupational Therapy program (OTD or equivalent)
• Previous experience in a Skilled Nursing Facility (SNF) required
• Strong clinical knowledge with a patient-centered approach
• Excellent communication, organization, and teamwork skills

• OTD or equivalent from an accredited Occupational Therapy program

• Patient evaluation and functional assessment expertise
• Treatment planning and implementation
• Therapeutic activities and adaptive strategies
• Patient and family education
• Interdisciplinary collaboration with PTs, nurses, physicians, and SLPs
• Documentation best practices and regulatory compliance
• Strong communication, organization, and teamwork
