Registered Nurse (RN) – Weekend Start of Care (Admissions), Home Health
Location:
Catonsville, Maryland
Compassionate and dedicated Weekend Start of Care Registered Nurse needed to serve adult and geriatric clients within their homes throughout Howard County, Maryland. The RN will perform per‑visit care for start‑of‑care/admission visits, working Thursday through Sunday from 8:30am to 5:00pm. This role supports a multidisciplinary team that provides rehabilitative care and skilled nursing to enable clients to remain at home.
Responsibilities- Follow a designated care plan in accordance with patient needs.
- Make home visits to clients in designated geographic territories.
- Administer medication, provide wound care, perform treatments and procedures as ordered.
- Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
- Execute and properly document doctors' orders.
- Perform client assessments as necessary.
- Coordinate case management and care.
- Accurately document observations, interventions, and evaluations using a state‑of‑the‑art tablet.
- Active Registered Nurse license in Maryland.
- Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home‑care experience strongly preferred; not required.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal and solid computer skills; experience with EMR preferred.
- Ability to travel to cases as assigned.
- Current Home Health and Oasis experience required for consideration.
- Salary: $55.00–$65.00 per point (depending on qualifications).
