Care Initiatives Hospice in Waterloo, IA is seeking a compassionate Hospice Admissions Nurse to facilitate patient transitions of care during admission and provide visit support to new admissions under our hospice care. This role requires working Monday through Friday, 9:00 a.m. to 5:30 p.m.
Hours may change based on admission needs. You will coordinate with Central Intake, Clinical Manager, Program Director and Community Representative to ensure seamless admissions and quality patient care.
