Position: Registered Nurse Case Manager New to Specialty

Job Summary and Qualifications

We are proud to offer our New to Specialty Case Management Program, designed specifically for Registered Nurses transitioning into Case Management who are ready to take the next step in their careers. This structured, supportive program delivers the education, hands‑on experience, and mentorship needed to confidently transition into the Case Management specialty and build a strong foundation for long‑term success.

Program Highlights:

  • Participants will complete a paid, immersive 6‑week onboarding program that combines hands‑on learning with guided support from experienced Case Management professionals. The program also includes 43 hours of Continuing Education Units (CEUs) that are applicable toward your nursing license.
  • To further support your professional growth and long‑term success, the organization fully covers the American Case Management Association (ACM) certification exam, along with all required study materials—removing financial barriers and investing in your development from day one.

This is an ideal opportunity for RNs looking to expand their career path, build specialized expertise, and transition into a rewarding role focused on care coordination, patient advocacy, and outcomes management.

Responsibilities

Your responsibilities will include:

  • Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
  • Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
  • Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals.
  • Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team.
  • Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient‑centered care.
  • Promoting HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction.
Qualifications
  • (RN) Registered Nurse
  • Bachelor’s degree, or Master’s degree, or Registered Nurse diploma
  • 3 years experience required
  • No travel required
