St. David's Medical Center seeks a Home Health Assistant Director to support leadership of our home-based care services. The role oversees clinical operations, staff supervision, and quality initiatives for neurological and complex-care patients transitioning from our full-service Austin hospital to home. Responsibilities include coordinating interdisciplinary care, ensuring regulatory compliance, mentoring RNs, optimizing workflows, and monitoring patient outcomes. The ideal candidate is an experienced RN leader committed to compassionate, patient-centered care and continuous professional development in a collaborative, high-performance environment.
Responsibilities
- Oversee daily clinical operations of home health services for post-acute and neurological patients
- Supervise, mentor, and schedule nursing and support staff to ensure safe, high-quality care
- Develop, implement, and monitor policies, procedures, and best practices for home health
- Ensure compliance with state, federal, Joint Commission, and payer regulations
- Coordinate interdisciplinary care plans with physicians, therapists, and case managers
- Monitor clinical quality metrics, patient satisfaction, and outcomes; implement improvement initiatives
- Manage workload, productivity, and resource allocation for cost-effective service delivery
- Participate in budgeting, staffing plans, and strategic growth of home health programs
- Lead staff education, orientation, and competency validation efforts
- Serve as clinical resource and escalation point for complex cases and family concerns
