Center Well Home Health is hiring a full-time Home Health Physical Therapist to deliver patient-centered care in patients' homes in the Cedar Rapids area. You will plan, implement and coordinate physical therapy, work with PTAs and aides, and document progress for physician evaluation and discharge planning.
Required:
current PT license, CPR certification, and teamwork experience;
Iowa City/North Liberty area knowledge is a plus.
