Occupational Therapist (OT) Northbrook Manor Care Center – Cedar Rapids, IA 6420 Council Street NE, Cedar Rapids, IA 52402

Northbrook Manor Care Center is seeking a compassionate and motivated Occupational Therapist (OT) to join our in-house rehabilitation team. This is an excellent opportunity for an OT who enjoys working with adults in a subacute rehabilitation and skilled nursing environment and is passionate about helping residents regain independence and return to meaningful daily activities.

Responsibilities
  • Evaluate residents' functional abilities, occupational performance, and rehabilitation needs.
  • Develop individualized treatment plans based on clinical findings, resident goals, and functional needs.
  • Provide skilled occupational therapy services to improve independence with activities of daily living and functional mobility.
  • Treat residents recovering from surgery, illness, injury, stroke, neurological conditions, falls, fractures, and other medical conditions.
  • Address ADLs and IADLs, including dressing, bathing, grooming, toileting, feeding, transfers, and other functional activities.
  • Provide therapeutic exercises, activities, neuromuscular re-education, and functional training as clinically appropriate.
  • Assess and address upper-extremity strength, range of motion, coordination, balance, and functional use.
  • Recommend and educate residents on appropriate adaptive equipment, assistive devices, and compensatory strategies.
  • Monitor resident progress and modify treatment plans as clinically appropriate.
  • Educate residents, families, caregivers, and facility staff regarding safety, positioning, adaptive techniques, and strategies to maximize independence.
  • Participate in care planning and interdisciplinary team meetings.
  • Communicate changes in resident condition and treatment recommendations to appropriate team members.
  • Complete timely and accurate clinical documentation.
  • Follow applicable professional, state, federal, and facility requirements.
  • Promote resident safety, dignity, independence, and quality of life.
Qualifications
  • Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program, as applicable.
  • Current Iowa Occupational Therapist license or eligibility for Iowa licensure.
  • Current CPR/BLS certification, as required.
  • Experience in skilled nursing, subacute rehabilitation, acute care, or another medically based setting is preferred but not required.
  • Strong clinical, communication, organizational, and documentation skills.
  • Ability to work effectively as part of an interdisciplinary rehabilitation team.
  • Compassionate, dependable, and resident-focused approach to care.

The ideal candidate is a patient-centered and clinically skilled Occupational Therapist who enjoys helping adults regain independence in the activities that matter most to them. You should be comfortable managing a diverse rehabilitation caseload, collaborating with nursing and other therapy disciplines, and developing practical treatment strategies that translate into meaningful improvements in residents' daily lives.
