St. Luke's Hospital Transitional Care Center is seeking a full-time LPN/RN for our 1st shift. As a member of our team, you will have the opportunity to work in a supportive and collaborative environment while providing high-quality care to our patients.
Job Responsibilities:- Provide direct patient care in a skilled nursing facility setting
- Assess patient needs and develop individualized care plans
- Administer medications and treatments as prescribed
- Collaborate with physicians and interdisciplinary team members
- Ensure compliance with all regulatory requirements
- Active LPN or RN license in the state of Iowa
- Previous experience in a skilled nursing facility preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in a fast-paced environment
