Life Space Communities, Inc., a leader in senior living services, is seeking a dedicated MDS Coordinator to join our dynamic team. In this role, you will be essential in enhancing our residents' quality of life, ensuring that our care strategies meet individual health standards. Your expertise in Resident Assessment Instrument (RAI) process and your ability to coordinate multidisciplinary care plans will support our mission to provide innovative and high-quality aging services across our communities.
Responsibilities
- Conduct and coordinate the RAI and MDS processes ensuring compliance with federal and state guidelines.
- Analyze resident data to ensure appropriate care plans are set and met.
- Collaborate with healthcare providers to tailor interventions based on resident assessments.
- Ensure that documentation and resident records are maintained accurately for state reports and audits.
- Train and support nursing staff on the MDS process, changes in protocols, and best practices.
Required Skills
- Proficiency in MDS 3.0
- Strong analytical skills
- Excellent communication and interpersonal skills
- Detail-oriented with a high level of accuracy
