Responsible for informing residents and families of available health and support services. Responsible for the development of contacts with service providers and agencies for resident referrals. Responsible for educating residents on available services and monitoring provisions of services. Responsible for fostering a continuum of care for residents.

  • Conducts initial and periodic reviews of residents to determine ability for admittance and feasibility of continued residence within community housing.
  • Monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory.
  • Provides limited case management needs for a resident when the general service community is not providing such service.
  • Provides CLC Medicare Advantage plan (Perennial) support with Permission to Contact (PTC).
  • Educates residents on service availability, application procedures, resident rights, etc. both individually and as a group.
  • Refers residents to volunteer support group programs with service organizations in the community.
  • Assists management in identifying residents who need assistance and may help in coordinating that assistance.
  • Maintains individual resident files and documents all pertinent information.
  • Support residents as active citizens in their communities, with influence, meaning, autonomy, belonging and an overall sense of well-being.
  • Other duties as assigned.

Qualifications Basic Qualifications & Experience
  • Experience in Social Work, Healthcare (behavioral / mental health), social services, Gerontology or related field.
  • Minimum 2 years experience direct service working with older adults, preferably in an assisted living or independent living community.
  • Able to effectively respond in crisis situations.
  • Must possess excellent customer service skills.
  • Able to read, write, and speak with English language
Working Conditions /

Physical Requirements
  • Light physical effort required by handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
  • Sits, stands, bends and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Interacts with families, volunteers, residents and numerous other staff members.
