We aim to provide local patient assignments whenever possible
.
- Perform comprehensive PT evaluations in patients' homes.
- Develop individualized treatment plans based on each patient's needs and goals.
- Provide therapeutic exercises, gait training, balance training, and mobility interventions.
- Help patients improve strength, function, and independence.
- Provide fall prevention and safety education.
- Teach patients and caregivers appropriate home exercise programs and mobility techniques.
- Communicate and collaborate with physicians, nurses, OTs, SLPs, caregivers, and other members of the care team.
- Monitor patient progress and adjust treatment plans as needed.
- Complete timely and accurate electronic documentation.
- Active Colorado Physical Therapist License
- Graduate of an accredited Physical Therapy program
- Strong communication and patient education skills
- Ability to work independently and manage your schedule effectively
- Valid driver's license and reliable transportation
- Compassionate, professional, and patient-centered approach
Home Health experience is preferred—but not required.
