Job Description
Position SummaryOn a flexible, as‑needed basis, provides clinical services to all residents residing at Communities of Christian Living Communities (CLC). These services will include skilled nursing, assisted living, and all shifts. Coordinates care and services based on resident needs and individual preferences as determined by ongoing assessment and evaluation. Performs duties with education‑based expertise, to provide a holistic and person‑centered model of care for residents.
Flex team members are expected to fill‑in for a minimum of 2 shifts per month. Most of these shifts are 12‑hour shifts. This position is considered PRN status and employees may sign up for multiple locations, dates, and shifts. With these expectations, the Community Staffing Team will communicate closely with team members toward meeting their preferences and CLC’s needs.
Essential Duties- Attend appropriate Resident Care conferences; maintain Resident Care Plans ensuring accuracy and relevancy.
- Recognize and identify signs and symptoms and use initiative and proper judgment in coordinating interventions.
- Communicate with other nursing staff, physician, pharmacist, resident, family member and others with need to know on resident’s status.
- Ensure protection of resident privacy and confidentiality.
- Ensure documentation is done via electronic medical record platforms, and is accurate.
- Prepare, administer, and document medications according to physician orders. Ensure that needed medications are available and expired medications are destroyed per protocol.
- Communicate any concerns, incidents, and progress of residents to all relevant parties.
- Document assessment findings, interventions, and follow‑up.
- Provide general oversight and coordination of services during shift. Report any concerns to the Director of Nursing, or appropriate supervisor.
- Complete all required onboarding trainings and periodic training updates thereafter.
- Perform other duties as assigned.
- Colorado State Licensed Practical Nurse license in good standing.
- Ability to work well with different teams, in different Communities.
- 1‑year experience in a long‑term care facility preferred, and coordinating care in a clinical environment.
- Experience in caring for residents with dementia a plus.
- IV Certification helpful.
- Must have computer word‑processing and database experience.
- 1‑year experience using electronic medical record platforms preferred.
- Must be able to read, write and speak the English language.
- Must possess excellent customer service skills.
Physical Requirements
- Moderate physical effort required by handling objects up to 50 pounds occasionally and/or up to 10 pounds frequently.
- Possible exposure to chemicals, blood‑borne pathogens and other infectious diseases.
- Sits, stands, bends, and moves intermittently during working hours.
- Is subject to frequent interruptions.
- Interacts with residents, family members, visitors frequently.
