Position: LICENSED PRACTICAL NURSE III - Rifle, CO

Responsibilities

  • Perform admission and daily assessments during clinical stay, identifying critical stays, incidents, emergencies, acute illnesses and behavioral changes with RN oversight.
  • Identify and implement appropriate nursing interventions, treatment care plans and physician referrals.
  • Document current, accurate and pertinent information regarding care and treatment provided to individual residents.
  • Indicate specific changes in status, medications and physician orders.
  • Detect abnormalities in a resident’s health status indicated by symptoms, vital signs and laboratory results and report information to physician.
  • Drive a State vehicle to conduct off‑site pre‑admission assessments or to transport a resident to the doctor or hospital when not taken by ambulance.
  • Have an RN co‑sign admission assessments.
  • Direct and implement nursing care to residents, ensuring compliance with accepted standards of nursing care, departmental and facility policies, and VA and State of Colorado regulatory requirements.
  • Direct subordinates to assure these standards are met.
  • Review and audit physician orders, MARS, CNA flow sheets, bathing schedules and BM sheets to determine compliance with policies and standards of care.
  • Competently administer medications to residents within scope of practice while observing the five rights of safe medication administration, resident rights, infection control standards, and appropriate nursing policies.
  • Document all medications given and justify scheduled medications not given.
  • Appropriately identify changes in a resident’s conditions that require assessment by a registered nurse.
  • Perform other duties as assigned.
Qualifications
  • Current, valid licensure as an LPN from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC).
  • Ability to follow patient confidentiality standards.
  • Excellent interpersonal and communication skills.
  • Ability to read, comprehend, and effectively communicate fluently in English both verbally and in writing.
  • Ability to follow care plans.
Conditions of Employment
  • Full background check required, including CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check.
  • Direct contact with vulnerable persons; requires full background checks.
  • Must possess a valid, non-restricted Colorado Driver’s License or a non-restricted U.S. Driver’s License for positions within 30 miles of the CO state border.
  • Out-of-state Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.
  • Must complete and pass TB test and any other vaccinations required by the facility.
  • State of Colorado residency required at the time of application and within the state throughout the duration of employment.
  • Eligibility for State of Colorado residency only; no relocation assistance provided.
