Horizons Care Center is a community of Volunteers of America National Services, a subsidiary of the Volunteers of America parent organization. Located at the base of Grand Mesa in Delta County, our setting offers a peaceful, small‑town atmosphere surrounded by orchards, scenic byways and easy access to outdoor recreation.

Pay Range: $39.59 - $46.55 per hour based on experience.

• Current licensure in the state of Colorado as a Registered Nurse.
• Experience in long-term care preferred.
• Technical/other training or better in Nursing.
• CPR/First Aid certification.

• Utilize the nursing process to plan and provide direct care to residents as assigned.
• Plan, direct, and carry out shift routines in the assigned nursing unit.
• Observe, direct, and evaluate activities of personnel as the person “in charge,” according to the Nurse Practice Act.
• Assume Building Charge responsibilities as directed by the DON or designee.
• Provide leadership and direction in the assigned unit.
• Serve on, participate in, and attend various committees and meetings to enhance interdisciplinary processes.
