Medical Provider
- Dual Certified Advanced Nurse Practitioner (APN)/ Family Nurse Practitioner (FNP) OR PA-C
Job Summary:
The Medical Provider will plan, direct, and evaluate client health care in ambulatory health services; provides supportive professional mental health and addiction services to the organization’s population, as well as treatment of chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses. Responsible for the appropriate evaluation and assessment of all clients in conjunction with prescribing clinically appropriate pharmacology. The Medical Provider provides accurate and appropriate medication assisted therapies and subsequent monitoring.
Reports to:
Chief Medical Officer (CMO) and Executive Director (ED)20 hours per week Duties/Responsibilities include but are not limited to:
Responsible for initial psychiatric evaluation and assessment of pre-existing conditions or conditions present in response to acute withdrawal;
Responsible for accurate and timely documentation in the Electronic Medical Records System within mandated time frames;
Prescribes and monitors appropriate medications for clients within the facility in accordance to all policy, procedures, and state and federal regulatory standards;
Works with nursing team on scheduling / client follow ups and medications prescribed
Responsible for the education of clients pertaining to psychopharmacology and medication assisted therapies as well as medication and narcotic use/misuse interactions and contra-indications;
Responsible for providing care and treatment to clients showing a change or decline in mental status, chronic or urgent medical conditions, and/or in acute or active withdrawal;
Completes client’s histories and physicals (H&P), assesses risk factors, and orders therapeutic and diagnostic tests;
Provides referrals for any issues identified throughout the course of the initial evaluation or ongoing care, and subsequently collaborates with any providers issuing care to program clients;
Consults with CMO on cases as needed;
Consistent communication with Nursing Manager and clinical team to ensure continuity of care and appropriateness of Level of Care (LOC).Participates in scheduled interdisciplinary team meetings;
On Call responsibilities to be determined imum Requirements:
Excellent verbal and written communication skills;
Must maintain active state license and DEA certificate in good standing;
Versatility, flexibility, and a willingness to work within changing priorities with enthusiasm;
Excellent interpersonal skills;
Must be able to work independently, efficiently and with accuracy;
Excellent organizational skills and attention to detail.
Exhibits a high degree of courtesy, tact, and poise when interacting with clients, families, and other personnel;
Ability to prioritize tasks.
Knowledge of Electronic Medical Records Systems and protocols;
