Position Summary: The Behavioral Health Navigator responds to behavioral health related calls with law enforcement to provide crisis de-escalation, intervention, assessment, linkage to appropriate services, and assistance in identifying client and family needs, gaps in service, and opportunities for assistance.

  • Works collaboratively with the police and law enforcement agencies to respond to individuals in crisis to prevent further criminal justice involvement, or hospitalization when possible and provide follow up to ensure service needs are met.
  • Provides risk assessment and intervention as part of the interdisciplinary team.
  • Refers to Consulting Clinician for in-depth assessment and evaluations.
  • Works closely with individuals to identify and address barriers that prevent them from getting ongoing resources and treatment and provides advocacy and care management for high acuity clients.
  • Assists patients in accessing community behavioral health resources.
  • Provides community outreach and education.
  • Serves as an information resource to co-workers, clients, and the general public.
  • Participates in job-related training sessions and seminars.
  • Provides excellent customer service to external and internal customers including but not limited to effectively listening and communicating, providing accurate information, offering solutions, assisting in resolution, and ensuring timely follow-up as needed.

  • Assesses client’s behavioral health status.
  • Attends required staff meetings.
  • Establishes and maintains partnerships with community partners.
  • Participates in job-related training and events.
  • Serves as an informational resource for law enforcement.
