Job Description
The Clinical Mental Health Professional at Centerville Day Treatment utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care for the care team, assess crisis situations to provide needed resources or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patients’ significant others, families, healthcare providers, insurers, community resources, and all other involved parties.
It provides care appropriate to the established plan of care for the assigned patient population.

- Diagnoses and treats various mental, emotional, and behavioral disorders, modifying care plan(s) to reflect changes in patient status.
- Refers patients to other services to improve their well‑being, including community services, treatment programs, support groups, psychiatrists, or other mental health providers.
- Works with the multi‑disciplinary team to develop a discharge plan to provide further care, services, and referrals to ensure continuity of care.
- Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in the area(s) of responsibility.
- Performs behavioral health consultation for patients and caregivers, conducts psychosocial assessments, and provides supportive, crisis bereavement, and other behavioral health interventions as necessary.
- Educates, communicates, and coordinates with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely discharge, and to identify and resolve delays and issues.
- May provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events.
- Master’s Degree in Social Work or related field from an accredited institution; degree will be verified.
- Current State Licensure as LCSW, LMFT, or CMHC.
- Excellent written and verbal communication skills.
- Computer skills, including email and word processing.
- Experience with patients, families, and treatment teams.
- Ongoing need to see and read information, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require verbal communication and rapid comprehension, particularly during emergency situations.
- Manual dexterity to manipulate delicate equipment with precision and accuracy, including frequent computer use and typing.
- Expected to lift and utilize full range of movement to transfer patients, bend to retrieve, lift, and carry supplies and equipment, often including heavy items.
- Requires walking and assisting with transporting/ambulating patients and obtaining and distributing supplies and equipment, including pushing and pulling gurneys and portable equipment that may be heavy.
- May be required to stand in a stationary position for an extended period of time.

Hourly rate is $35.25 - $54.39, dependent upon experience.
