• Ensure that all activities performed align with the vision of Elara Caring’s board, executive team, and Home Health leadership.
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
• Assess assigned caseload of home health care patients and families to identify physical, psychosocial, and environmental needs through documentation, clinical records, interdisciplinary reports, and on‑site evaluations.
• Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
• Assess patient needs and obtain data on physical, psychological, social, and spiritual factors influencing health status, incorporating that data into the plan of care.
• Make initial evaluations and re‑evaluate nursing needs during each visit.
• Make follow‑up visits according to patient, family, and facility needs.
• Communicate significant findings, problems, and changes in health condition or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
• Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members.

• Current, unrestricted state RN license.
• 1+ year experience in a clinical care setting as a nurse.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Ability and willingness to travel within branch/office coverage area (50% of time).
• Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Associate’s or Bachelor’s Degree in Nursing preferred.
• Previous Home Health or Hospice experience preferred.
• Experience with Home Care Home Base preferred.
