Job Details:
Outpatient Services l Centerville l Part-time l Days.

• Identification and assessment of patients and families who have social, psychological and/or environmental needs related to the impact of cancer diagnosis, treatment, or side effects.
• Interviewing patients and families, developing plans for intervention addressing agreed‑upon priorities.
• Synchronizing appointments and referring patients to needed community resources.
• Assisting patients/families in improving or restoring their capacity for social functioning.
• Use a proactive approach to facilitate patient care through staff huddle and collaboration with the care team.
• Ensure psychosocial needs are met and understood.
• Other duties as assigned.

Bachelor s degree in Social Work required for SW I

Ohio, United States:
Social Worker license required. OSW-C (oncology social worker certification) preferred.

A minimum of 2 years  experience in acute care and/or oncology setting preferred.

• Clinical experience in health care setting and/or community agencies.
• Awareness of developmental needs over the age span of adolescent to elder.
• Work well in interdisciplinary and independent settings.
• Communicate verbally and through writing.
• Coordinate work effectively with other hospital disciplines and locations.
• Recognize and understand the dynamics of individuals from diverse populations.
• Solve problems through the analysis of information.
• Network with community agencies.
• Provide education to the community and/or medical setting regarding social issues.

Experienced in basic computer skills and electronic medical records.

Position is in an outpatient Neurologic therapy clinic working with patients who have new and/or existing neurologic disorders.
