Location: Dayton and surrounding areas
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Only 2 holiday on‑call days per year)

As a Hospice RN Case Manager
, you will:
- Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
- Deliver holistic patient care
, addressing physical, emotional, psychological, social, and spiritual needs - Collaborate with the interdisciplinary care team
, including physicians, social workers, and chaplains - Coordinate care with attending physicians and external providers
- Conduct routine patient visits and update individualized plans of care
- Provide end‑of‑life care
, including clinical pronouncement and family support - Maintain accurate, timely documentation in compliance with state, federal, and company standards
- Supervise and oversee care delivery by Home Health Aides (HHAs)
- Active Registered Nurse (RN) license in Ohio (or compact state if applicable)
- Graduate of an accredited nursing program
- Current CPR and First Aid certification
- Knowledge of quality assurance and hospice care standards
- Valid driver’s license and auto insurance
