Position SummaryWe are seeking a Licensed Nurse Practitioner (NP) to join our interdisciplinary hospice care team in Dayton, OH. This role is responsible for assessing patient eligibility, coordinating care, and supporting patients and their families through compassionate, high-quality hospice services.
Key Responsibilities- Determine patient eligibility for hospice services based on State and Federal guidelines
- Conduct comprehensive assessments, including physical, psychological, social, and spiritual needs
- Collaborate with physicians and interdisciplinary team members to develop and coordinate individualized care plans
- Initiate communication for admissions and recertification of care
- Provide routine visits to reassess patient condition and update care plans
- Utilize clinical judgment to optimize patient outcomes and quality of life
- Support patients and families through emotional and end-of-life care needs
- Graduate of an accredited nursing program
- Minimum 2 years of experience as a professional nurse
- Current BLS certification required
- Strong clinical decision-making and assessment skills
- Ability to provide care in home-based settings
- Experience handling emotional and sensitive family situations
- Excellent interpersonal, verbal, and written communication skills
- Valid driver’s license and proof of insurance
