Licensed Practical Nurse (LPN) – Hospice (PRN / Part-Time)

Location: Dayton and surrounding areas
Schedule: 3 Twelve-Hour Shifts Per Month (Flexible / Night Shift)

• Only 3 shifts per month (12-hour shifts)

• Provide direct patient care in accordance with State Nurse Practice Act and hospice standards
• Perform comprehensive patient assessments and support individualized plans of care
• Deliver skilled nursing services focused on comfort and quality of life
• Administer and monitor pain and symptom management interventions
• Educate patients and families on diagnoses, disease progression, and care plans
• Document patient care accurately and timely in the electronic medical record (EMR)
• Collaborate with the interdisciplinary care team to ensure coordinated hospice care
• Evaluate patient response to treatment and adjust care as needed

• Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
• Current LPN/LVN license in the state of practice
• Minimum 3 years of nursing experience (hospice, home health, med‑surg, or community health preferred)
• Current BLS certification
• Strong clinical assessment, communication, and organizational skills
• Valid driver’s license

• Understanding of hospice philosophy and end‑of‑life care
