Four days off each week for improved work life balance
- Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards
- Conduct comprehensive nursing assessments and implement individualized plans of care
- Deliver skilled nursing services focused on comfort dignity and quality of life
- Administer and monitor pain and symptom management interventions
- Educate patients and families on disease progression care plans and symptom control
- Document patient care accurately and timely within electronic medical records EMRCollaborate with the interdisciplinary team to ensure coordinated holistic care
- Evaluate patient responses and adjust care plans as needed
- Graduate of an accredited Practical Vocational Nursing LPNLVN program
- Current LPNLVN license in the state of Ohio
- Minimum 3 years of nursing experience hospice home health med surg or community health preferred
- Current BLS certification
- Strong clinical communication and organizational skills
- Valid drivers license reliable transportation and proof of insurance
- Understanding of hospice philosophy and end of life care
- Experience in hospice or palliative care
- Familiarity with home based care and EMR systems
