Salary: $66,300/annually

Schedule:

9AM-5PM, Monday-Friday

320 Carleton Ave. in Central Islip NY

What Drives the Core Purpose of This Job

The Case Manager Supervisor is responsible for the supervision and training of Case Managers, interns, and volunteers of the Community Guardianship Program.

Key Responsibilities
  • Assist in the training and supervision of Case Manager, interns, and volunteers.
  • Support program staff to determine best course of treatment, placement, and ongoing care of clients.
  • Identify and bring resources to the program through community outreach and liaison with providers.
  • Oversee the fact-finding process and aid staff to develop a service plan for Guardianship clients.
  • Coordinates timely submission of all program reports, Initial Report, Annual Reports and court updates for clients.
  • Collaborate with program Bookkeep and In-house Counsel on cases.
  • Facilitate case conference on a weekly basis to ensure all program staff is updated on new client issues, resources, court dates, reports etc. and to facilitate decision making.
  • Maintain a caseload and perform all Case Manager responsibilities such as; home visits, telephone reassurance, advocacy for clients, transport/escort clients in own vehicle if necessary to and from appointments when needed to obtain emergency resources or to complete applications, apply for government/private benefits with Medicaid, Social Security, VA, pension companies, and retirement institutions, perform property management duties, marshal assists and make pre-funeral arrangements for individuals.
  • Collaborate with the Program Director in oversight of case file review and implementation of program policies and procedures.
  • Perform all other relevant duties as assigned by Supervision and EAC NETWORK Administration
  • Key Knowledge, Skills, and Abilities Required to Excel in This Role
  • Bachelor's Degree in social or behavioral sciences required.
  • Must be proficient in office and computer applications or other automated systems such as spreadsheets, Microsoft Office applications, Calendar program, and e-mail and/or database software skills necessary to perform job responsibilities.
  • Minimum of two years of relevant work experience preferred.
  • Excellent interpersonal and communication skills, both oral and written.
  • Ability to motivate clients.
  • Be available for emergencies that may occur after normal business hours.
  • Must have access to automobile and possess valid NY State Driver's License.
