We are seeking a Registered Nurse
- Home Health to join our team at Signature Healthcare  this role, you will provide essential nursing care to patients in their homes, ensuring their health and comfort while promoting independence. Your expertise will be pivotal in delivering high-quality healthcare solutions tailored to each individual's needs.

Responsibilities

• Conduct comprehensive patient assessments to develop personalized care plans.
• Provide skilled assessment and interventions.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and their families on health management and wellness practices.
• Coordinate with interdisciplinary teams to ensure continuity of care.
• Maintain accurate documentation in compliance with HIPAA regulations.

Requirements

• Current Registered Nurse (RN) license in the state of Oregon
• Proven experience in acute care or home health nursing preferred.
• Strong patient assessment skills and critical thinking abilities.
• Excellent communication skills for effective patient interaction.
• Knowledge of HIPAA regulations and commitment to patient confidentiality.
• Valid driver's license
• Must maintain current CPR/BLS certification
