Overview
Registered Nurse plans, organizes and coordinates home care services with an emphasis on community health education. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
Responsibilities- Develops and initiates the plan of care and leads ongoing interdisciplinary assessment and development of individualized plans in partnership with patients, representatives and caregivers.
- Initiates appropriate preventative and rehabilitative nursing procedures ordered by the physician and provides other ordered services.
- Provides patient, caregiver and family counseling and education.
- Prepares and maintains clinical notes and updates the primary physician at least every sixty days.
- Communicates with the physician responsible for the home health plan of care and other health care practitioners.
- Identifies discharge planning needs as part of the care plan and implements them prior to patient discharge.
- Coordinats with community health personnel to align the care plan.
- Participates in the HHA’s quality assessment and performance improvement program and in-service training.
- Participates in on‑call duties as defined by the procedure.
- Ensures arrangements for equipment and other necessary items and services are available.
- Instructs, supervises and evaluates home health aide care provided every fourteen days.
- Supervises Licensed Practical Nurses, Licensed Vocational Nurses and/or Home Health Aides or Certified Nursing Assistants as directed.
- Graduated from an approved nursing school and licensed in the state of practice.
- Minimum two years of experience, at least one year in public health or home care nursing preferred.
- Valid, current CPR certification.
- Valid driver’s license, automobile insurance and access to a vehicle for agency travel.
- Self‑directed, able to work with minimal supervision.
- Excellent observation, problem‑solving, verbal and written communication skills.
- Ability to organize and prioritize workload independently.
- Responsible for supervising home health aides.
- Physical ability to lift, position, or transfer patients and move supplies.
- Ability to walk, stand, bend, kneel, crouch and reach for a prolonged period.
- Adequate visual acuity and hearing to perform required nursing skills.
