Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• PT license
• At least 1 year home health experience
• Minimum Education Requirement:
  Doctoral

Assignment Details

• Coverage area includes Centralia and surrounding areas
• No on call or weekend requirement
• Mileage reimbursed between patient visits, from first patient to last patient, excluding travel from home to first patient and last patient to home
• Dress code: navy blue scrubs, navy blue top with khaki shorts
