Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team. Compensation:
Local Pay: $55.00 - $70.00/hour (W-2) Travel Pay Package:
Up to $2,800/week gross (includes weekly non-taxable stipend)

Key Responsibilities:

• Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments, while developing individualized treatment plans, improving patients' functional mobility, and meeting a productivity expectation of 30 points.
• Deliver evidence-based therapy interventions, monitor and document patient progress accurately in Homecare Homebase (HCHB)/Point Care, collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards across the assigned coverage areas.
