Monday–Friday, 11:00 AM – 7:00 PMSouth Creek Post Acute is seeking a dedicated Admissions Nurse to join our team. This position plays a key role in coordinating patient admissions and ensuring a smooth transition into our facility.

Position Details:

Schedule:

Monday–Friday, 11:00 AM – 7:00 PMLicense: RN preferred; LPN will be considered

Pay Range: $40–$55 per hour, based on credential and experience

Key Responsibilities:

Coordinate and complete patient admissions

Review and verify admission documentation

Communicate with hospitals, families, and interdisciplinary team members

Ensure compliance with facility policies and regulatory requirements

Provide a positive admission experience for new residents

Qualifications:

Active RN or LPN license in good standing

Strong communication and organizational skills

Experience in post-acute, skilled nursing, or admissions preferred

Ability to work independently and collaboratively
