Position Summary
Under the direction of the Hospice Director or designee and on order of the physician, the RN is accountable for direct and indirect quality care for patients and families of all ages, following the nursing process and the Nurse Practice Act. The RN manages a caseload of patients living within the hospice service area, coordinates care activities with the healthcare team, promotes patient comfort and assists in achieving a peaceful death.
The role requires confidentiality, attendance and punctuality, and professional development as part of nursing practice.
- Manage a nursing caseload as assigned, including patients in diverse environments and geographic areas.
- Perform nursing assessment and develop the care plan.
- Initiate admissions and plan of care.
- Collaborate with physicians and team members to develop interdisciplinary hospice plan.
- Continuously evaluate patient and family psychosocial, spiritual, and physical needs; initiate timely referrals.
- Teach and counsel patient and caregivers regarding care, preferences, and limitations, considering cultural, religious, language, and communication aspects.
- Maintain clinical, progress, and itinerary records to meet JCAHO and federal regulations.
- Direct care provided by Home Health Aides, respite aides, and volunteers with verbal and written instructions.
- Under case manager direction, update interdisciplinary plan every 14 days or as needed.
- Inform physicians, team members, and other parties of changes in patient condition.
- Rotate through weekend, holiday, and 24‑hour call per supervisor assignment and PAMC contract.
- Perform procedures and treatments per organizational policies.
- Participate in continuous quality improvement and other quality management activities.
- Precept new staff and/or students as assigned.
- Demonstrate professional nursing practice advancement through community or professional activity.
- Meet or exceed department productivity standards as defined by patients per caseload.
- Maintain advanced knowledge in end‑of‑life care: pain and symptom management, bereavement, ethics, hospice philosophy, community resources, reimbursement.
- Graduate of an accredited school of nursing.
- Minimum 2 years’ RN experience.
- Preferred: hospice or home health experience.
- Within 30 days of hire:
National Provider (NPID) and Basic Life Support (BLS) – American Heart Association (vendor‑managed). - Upon hire:
California Registered Nurse license (vendor‑managed).
Regular standing, walking, and manual dexterity; ability to lift up to 50 lbs. Requires close and distance vision, color and peripheral vision, depth perception, and focus adjustment. Exposure to bodily fluids, infectious diseases, and related conditions is expected. Routine use of standard medical equipment and tools associated with clinical care.

Compensation & BenefitsCompensation range: $50.18 – $77.91 per hour. Additional compensation may include shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
