Memory Care LPN

Licensed Practical Nurse

Full-time / Non-exempt / Pay based on experience

The Licensed Practical Nurse in the Memory Homes is responsible for assisting the Director of Memory Homes and team members to carry out procedures to provide services to the residents of the Memory Homes.

Essential Functions

• Support Memory Homes operations and Household Team Members
• Participate in care planning, including keeping resident assessments up to date, such as RASPS, MMSE, and resident interviews.
• Participate in on-call rotation and fill gaps in the daily schedule to ensure coverage
• Administer medications and provide daily nursing care, including emergency response.
• Ensure accurate resident documentation and maintain Electronic Health Records.
• Coordinate resident appointments, transportation, and serve as administrator designee when needed.
• Oversee meal preparation, uphold safety and cleanliness standards, and provide individualized, resident-focused care.

Qualifications:

• High School Diploma, or equivalent, required
• Graduate of a state-approved school of nursing and currently licensed in good standing as a practical nurse in the state of PA, required
• Holds a current CPR certification, required
• Competent nursing skills as required by the PA Nurse Practice Act, required
• Strong computer skills, including the use of an electronic health record (EHR) system, required
• Strong communication and interpersonal skills, required
• Positive attitude with a focus on promoting teamwork and maintaining a supportive work environment, required
• Previous experience in a healthcare setting, preferred
• Previous experience in a supervisory role, preferred

Shift:

• Full-time, Non-exempt
• Monday - Friday, 7 am-3:30 pm with occasional weekends
• On-call as needed
