PositionSummary
This position is dedicated to providing post-crisis stabilization and facilitating connections to ongoing care tailored to each client’s needs. Responsibilities include conducting formal assessments, identifying treatment requirements, and developing comprehensive safety plans in collaboration with clients and their families. The role also involves working closely with other programs, such as Mobile Crisis Response (MCR) to ensure clients receive the necessary support and resources.

- Non-licensed - $3,000
- Licensed - $4,000
- Bachelor’s degree in Psychology, Counseling, Social Work, or other related field, or equivalent experience and training
- Must qualify at a minimum as a MHP per Illinois Department of Mental Health Rule 132
- Excellent written and oral communication skills
- Ability to quickly establish rapport with clients in crisis
- Computer proficiency required
- Ability to navigate and complete documentation in the electronic healthcare record
- Ability to accurately complete required documentation within the prescribed time frames
- Self-starter with the ability to work independently and collaboratively within a team
- Ability to manage multiple deadlines
- Valid driver’s license and the ability to meet agency insurance requirements
- Ability to transport clients as needed

- Work scheduled hours and any additional hours deemed necessary by leadership. Some variation in scheduled hours may be required to meet company needs.
- Participate in the on-call system as required.
- Provide crisis stabilization services to meet the needs of the clients in community settings to best serve those clients.
- Respond to behavioral health crises with appropriate crisis intervention strategies.
- Observe clients in their natural environment and provide coaching to clients and their families on the effective use of their crisis safety plan.
- Model positive coping skills and response patterns for clients and their families.
- Redirect client behaviors that begin to escalate and educate clients on how to respond to and reduce environmental stressors and stimuli when feeling overwhelmed.
- Completion of required assessments and required documentation to link clients to longer term care programs
- Utilize Electronic Health Record to open clients into services and to audit charts for necessary documentation.
- Maintain a caseload of crisis clients with the goal of linking the clients to long term care.
- Ensuring crisis program fidelities are met regarding discharge planning, MCO communication, and client staffing through monitoring documentation and performing these tasks as needed.
- Communicate effectively with internal and external stakeholders through consultations to receive necessary information to coordinate care with outside providers as well as to ensure accurate discharge planning is occurring.
- Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
- Understand and comply with the principles established by the Rosecrance Corporate Compliance Program and Code of Conduct.

Base Pay:
Bachelor’s $50,835.20 a year, Master’s (Qualified Mental Health Professional) $57,761.60 a year
