Ambulatory Care Pharmacist
- Clinical Consultation: Provide comprehensive medication management (CMM) for complex chronic conditions, utilizing evidence-based protocols to optimize pharmacotherapy and patient outcomes.
- Interdisciplinary Collaboration: Serve as the clinical pharmacy expert within a care team model, delivering real-time recommendations to providers on medication prescribing, administration, and monitoring.
- Quality & Safety: Conduct meticulous medication reconciliations and clinical data analysis to ensure high-quality, cost-effective pharmaceutical care across multiple clinic sites.
- Patient Advocacy: Lead patient education initiatives in the outpatient setting to improve medication adherence and health literacy.
- Operational Agility: Support diverse patient populations by providing flexible, high-level clinical coverage across multiple ambulatory clinics as assigned.
Education
- Doctor of Pharmacy (Pharm
D) - or Bachelor of Science in Pharmacy with at least 3 years of experience
- Registered Pharmacist (RPh) | Illinois Department of Financial and Professional Regulation (IDFPR)
- Clinical Pharmacist of at least 2 years
- Board Certification in Ambulatory Care and/or completion of an Ambulatory Care residency preferred
- Clinical Analysis: Expert in analyzing complex clinical data to optimize pharmaceutical outcomes and solve intricate patient-care problems.
- Regulatory Compliance: Comprehensive knowledge of Federal and State pharmacy laws, including DEA regulations and state-specific IDFPR standards.
- Interdisciplinary Collaboration: Proven ability to maintain a credible professional image while working alongside physicians to improve care team efficiency.
- Communication: Skilled in clear, professional oral and written communication with both diverse patient populations and healthcare providers.
- Continuing Education: Committed to maintaining current knowledge of therapy guidelines, drug recalls, and market changes (new medications/removals).
- Technical Proficiency: Advanced use of MS Office Suite (Excel, Word, PowerPoint) for clinical reporting and data management.
Core Clinical Responsibilities
- Comprehensive Medication Management (CMM):
Conducts detailed medication reconciliations and reviews to resolve discrepancies, duplications, and dangerous interactions. - Clinical Interventions & Monitoring:
Performs therapeutic interventions, including adjusting, initiating, or discontinuing medications under with physicians. - Polypharmacy & ADR Management:
Works to reduce unnecessary drug use (polypharmacy) and proactively identifies or minimizes adverse drug reactions (ADRs) to improve patient safety. - Disease State Management:
Manages complex chronic conditions such as diabetes (e.g., insulin titration), hypertension, and anticoagulation through dedicated clinic appointments. - Specialized Referrals:
Identifies when patients require specialized services and refers them to pertinent clinics like anticoagulation or behavioral health.
- Service Development:
Involved in the design, implementation, and evaluation of patient care services within ambulatory settings. - Subject Matter Expertise:
Serves as a drug information resource and subject matter expert for healthcare providers and clinical committees (e.g., Pharmacy & Therapeutics). - Regulatory Compliance:
Ensures all practices meet governmental and commercial regulations, including maintaining current knowledge of the Illinois Pharmacy Practice Act. - Medication Utilization Reviews (MUR):
Performs audits and reviews to evaluate drug utilization.

Educates patients on medication use, potential side effects, and lifestyle changes to improve compliance. - Mentorship:
