Job Description
Schedules therapy professional field staff, receives referrals, communicates scheduling and orders with client care staff.
Receives and processes all requests for therapy services. Distributes therapy referrals for assignment. Communicates with the field staff to coordinate professional and ancillary services.Schedules and maintains weekly calendar of all therapy services delivered. Reviews assignments as appropriate with the nursing supervisor, professional field staff and support staff.Provides back up support to in take coordinatorEnsures that all clients receive services as requested.Works cooperatively with the professional field staff to communicate client status and nursing supervisor. Communicate with and receives instructions from nursing staff on physician’s orders regarding the client’s condition.Communicates effectively with clients and families regarding schedule and staffing changes as requested.Accurately reviews and verifies activity logs against weekly scheduler and clinical records submitted.Answers the phone and route calls, information and people to the appropriate staff.Performs other duties as requested by the nursing supervisor and support staff.**** Previous Home Health experience is preferred****
