Physical Therapist (PT) - Outpatient Therapy

Overview

Evaluates and treats patients of all ages, modifying plan of care as appropriate. Serves as a role model in delivery of professional services and as a clinical resource for staff and students.

New grads, with less than 1 year of experience post‑graduation and licensure: $5,000

Candidates with at least 1 year of experience post‑graduation and licensure: $7,500

Relocation available if moving 50+ miles.

• Education Level: Bachelor’s Degree in Physical Therapy
• Licenses/Certifications:
  • Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR)
  • Basic Life Support (BLS) within 30 days – American Heart Association (AHA)
• Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.

• Perform patient transfers with assistance as needed from bed, chair, and floor, demonstrating safe and appropriate body mechanics.
• Treat patients and colleagues with dignity and respect, showing empathy and staying fully engaged.
• Design an appropriate plan of care integrating goals, treatment, outcomes, and discharge plan; adjust based on patient response and evidence.
• Educate patients, families, caregivers, students, and other health care providers with effective teaching methods.
• Evaluate clinical findings to determine physical therapy diagnoses and outcomes of care.
• Perform examinations, selecting reliable and valid methods relevant to the chief complaint, screening results, and history.
• Produce documentation in accordance with the Clinical Process Policy (CTSCP
  67).
• Screen patients to determine effectiveness and need for therapy services, if appropriate.
• Use support personnel according to legal standards and ethical guidelines; monitor and provide feedback.
• If applicable, complete annual education as assigned to comply with DNV Hip and Knee Replacement Program requirements.

• Evaluate and implement intervention plans using functional activities, considering functional and emotional needs.
• Modify approach in response to patient behavior and performance.
• Utilize standardized outcome measures uniformly to accurately reflect functional performance.
• Complete documentation according to regulatory and organizational standards.
• Collaborate with patients, families, and significant others throughout rehabilitation and provide education/family training as needed.
• Supervise PTA and rehabilitation technicians in accordance with regulatory agencies.
• Communicate patient performance, status changes, activity/transfer recommendations, and discharge disposition to the medical team.

• Compensation range: $36.74 per hour – $63.19 per hour. Actual offer based on experience, qualifications, location, training, licenses, shifts, and compensation model.
