Overview

The Home Services Social Worker identifies the psychosocial needs of patients and families through assessment. Social work interventions range from resource support identification and acquisition (including community support, financial and environmental enhancement) to short‑term counseling and emergent crisis intervention. Social services are provided as part of a collaboration with interdisciplinary teams. Hours may vary depending upon census and program need.

Qualifications
  • Licensed Clinical Social Worker (LCSW) – Illinois Department of Financial and Professional Regulation (IDFPR)
  • Driver's License – Illinois Secretary of State (ILSOS)
  • Proof of Auto Insurance – varies
  • Licensed Social Worker (LSW) – Illinois Department of Financial and Professional Regulation (IDFPR)
  • Master’s Degree in Social Work (required)
  • Work Experience in hospice care (required)
Responsibilities

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation. Develop a bereavement plan of care to address family member/caregiver needs. Interpret and communicate patient/family faith and cultural traditions. Identify and communicate when spiritual/religious beliefs may impact the physical and psychosocial care provided by other team members.

Educate patients and family members in a manner that overcomes barriers; match their learning capabilities and meet fundamental needs. Complete visit notes and orders in a timely manner according to policy. Correct care plans in a timely manner. Document psychosocial patient/family assessments, financial assessment, and MSW interventions within the electronic medical record. Implement a social work plan that results in enhanced family systems, increased utilization of community resources, maximization of medical benefits, improved care environment, dignity for the dying patient, maximized coping skills, and support for cultural beliefs and values.

Evaluate the effectiveness of the social work plan, modify interventions as indicated, and facilitate patient decision‑making and communication of self‑determined life care decisions. Practice sensitively to the needs of patients and families, with daily documentation reflecting an understanding of palliative/comfort philosophy versus aggressive/curative treatment. Identify and respond to indicators of imminent death, address patient/family needs at the time of death, demonstrate understanding of Medicare Hospice Benefit, utilize Memorial Funds appropriately, assist with transport barriers, and provide referrals for food, clothing, assistance with utilities, Medicaid/Community Care/SSDI applications, and elder abuse/neglect.

Educate patients and caregivers on advanced directives, including living will, HCPOA, and POLST forms.

Compensation and Benefits

The compensation range for this position is $27.36 per hour – $45.69 per hour. The actual compensation offered to a candidate will depend on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked, and compensation model.
